Advanced Practice Provider: Community Health Mobile Program

Location: Indian Hills Senior Living Community & Collinwood Health Center
Biweekly Hours: 80.00
Shift: 8-5
Summary:
The MetroHealth Community Health Center is looking for an APP to support our growing programs. We are seeking a provider who can provide care to patients across the lifespan with primary locations being in Collinwood and Euclid. Community Outreach within the FQHC-LAL continues to grow, and your clinical location may change daily (schedule known in advance) at a community partner clinic (ex: food bank) or a mobile unit (ex: homeless shelter). This APP will serve patients across the lifespan (pediatric through geriatric), offering comprehensive primary care, telehealth services, chronic disease management, and care coordination with our integrated team. - Family Medicine certification and experience required.
- 1 year of APRN experience required.
- 2 years of APRN experience preferred.
