Full Job Description

We are looking for an outgoing, dynamic and knowledgeable individual to be our Forensics Case Manager. This person will provide comprehensive behavioral health services to Lake County’s criminal justice population. This person acts as a liaison between mental health, substance abuse, and criminal justice systems, promoting rapid access to services during incarceration, and continuity of services in the community on release. This person offers case management services within the home and community setting. This person will also provide trauma informed case management services, including but not limited to system monitoring and ongoing assessment of needs, to clients and/or their family systems in the homes and the community. 

More specifically, this role is responsible for:

  • Conducting ongoing assessment of client’s mental health needs and related needs for improving or maintaining successful community living skills
  • Providing support to clients and families in crisis situations, and educates clients and families in crisis management.
  • Working collaboratively with clients to develop individual service plans that address client needs, incorporate client preferences for services, work toward client-selected goals, and strive towards decreasing recidivism with the criminal justice system.
  • Monitoring the implementation of the individualized service plan and progress toward goals.
  • Providing direct service support to clients in their homes, communities, jail, as well as in agency offices, to promote client access to appropriate services.
  • Making referrals to appropriate service systems, and serves as a client advocate in navigating various health, education, housing, social service and criminal justice systems to obtain necessary supports.
  • Maintaining regular contact with client for symptom monitoring, and reports/consults with other providers re: changes in symptoms and mental status.
  • Making jail visits, home visits throughout the community and transporting clients in own car.

Position Qualifications:

  • High School diploma or Associates degree in social work, counseling with three years experience. Bachelor's degree preferred.
  • QMHS/QBHS within 15 days of hire (employer provided)
  • Experience and knowledge of mental illness and substance abuse recovery in a work setting
  • Experience with mental health in corrections setting highly preferred
  • Excellent interpersonal, verbal and written communication skills essential
  • Valid Driver's License
  • Must have an acceptable driving record (4 pts. or less), reliable transportation, and carry automobile insurance with minimum liability limits of $100,000/$300,000 and property damage of $50,000.
