Job Summary

This position is responsible for providing project management and leadership to support strategic initiatives across business units for Post-Acute Care Services (PACS), including the management, performance, and completion of multiple concurrent projects. Will lead project management activities and serve as a subject matter resource for PACS. Duties will include managing large, complex project deliverables that require collaboration across internal and external stakeholder groups. Will coordinate project analysis, documentation, and logistics using industry standards, policies/procedures, plans/specifications, and quality/safety requirements to help drive decision-making. Effectively applies logic, principles, and analytics to aid in project decision-making. Assesses complex problems, draws conclusions, and provides value-driven solutions.

Responsibilities*

Characteristic Duties and Responsibilities:

Essential

  • Works with PACS leadership to prioritize projects of systems and processes, including

regular incorporation of partners (patients and family members, staff, faculty) in the design of products, and services.

  • Acts as a liaison between various departments to ensure that all project activities are in support of the health system?s overall goals and objectives. 
  • Proactively integrates the philosophy and practice of lean thinking and continuous quality improvement to meet the desired goals and metrics across PACS.
  • Responsible for consistently applying knowledge, skills, tools, and techniques to meet project requirements: scope, budget, quality, time, communications, change management, risk management, and stakeholder management.
  • Responsible for the project planning, design, build, implementation, and close phases, and for providing a single point of contact for all projects.
  • Responsible for project tracking, reporting, and status meetings, ensuring successful implementation of project activities with stakeholders.
  • Identifies, assesses, documents, and resolves project issues, risks, decisions, and dependencies; escalates to appropriate stakeholders when necessary.
  • Assigns tasks across project teams and stakeholders, and maintains excellent working relationships. Coaches and mentors project teams and stakeholders.
  • Conducts meetings, effectively communicates, and manages change with stakeholders.
  • Acts as a liaison between various departments to ensure that all project activities are in support of overall project goals and objectives.
  • Effectively coordinates and facilitates planning and deliverables for projects.  Prepares written communications and presentations for a variety of stakeholders, including senior leadership. 
  • Ensures alignment of projects to institutional policies, standards, and benchmarks and ensures adherence to project scope, budget, time, and deliverables.

    • Actively engaging and building rapport and trust with the patient and family, as applicable,

  • Collaborates with other members of the team, including the patient and family, to achieve the ideal patient care experience.

Other Related Duties:

  1. Complies with all University of Michigan, Michigan Medicine and departmental policies, procedures, standards and expectations.
  2. Supports the department?s commitment to continuous improvement activities and principles through positive and respectful contribution in all problem-solving and change processes at Post-Acute Care Services.
  3. Performs other duties as assigned.
Supervision Received From:     Director of Home Care Nursing 

Supervision Exercised:            Functional and Administrative supervision is exercised.

Physical Requirements: 

The employee will be able to perform the assigned duties and responsibilities with or without reasonable accommodations: 

  • Ability to walk or stand for long periods of time.
  • Ability to communicate/exchange information; read, write, speak and comprehend English.
  • Functional visual acuity and hearing for accurate assessment and recording of patient observations and information in clinical records.
  • Manual dexterity to perform required procedure activity proficiently and safely.
  • Ability to move up to 10 pounds. 
  • Ability to work in a sedentary position for long periods of time, fine motor coordination/dexterity, and may require reaching, bending, twisting, stooping, moving about.

OCCUPATIONAL HAZARD INFORMATION: 

Staff members may have routine exposure to all chemical inventory, inclusive of hazardous materials, housed at all work sites for Post-Acute Care, including but not limited to: the office, hospital, clinics, vehicles and warehouse.  Chemical inventory reference materials are available to all staff in the form of hard copy and on-line Material Safety Data Sheets.  

Staff may carry out assignments in patient?s homes, hospitals, clinics and Post-Acute Care offices.  Staff may drive personal or University owned vehicle and walk on uneven surfaces to these various work site locations in all weather conditions on a regular basis.

Required Qualifications*

?    Bachelor?s degree in a healthcare related field (e.g. Administration, Business, Engineering, and/or Health Sciences) or equivalent combination of education and experience.  
?    3-5 years of healthcare project, clinical, facilities, and/or operations management experience. 
?    Knowledge of theoretical and practical aspects of project management, techniques, and tools. Demonstrated ability to lead all project management processes from project planning to closeout. 
?    Ability to analyze, compare, and evaluate various courses of action and have the authority to make independent decisions on matters of significance, free of immediate directions, within the scope of their responsibility.
?    Highly analytical and has the ability to translate and communicate the analysis into actionable information for problem-solving and decision-making at all levels within organization.
?    Demonstrated ability to independently manage change and solve problems in an ambiguous environment. 
?    Demonstrated ability to lead teams and achieve results. Strong team orientation and ability to manage in complex organizations. 
?    Ability to work collaboratively and demonstrate flexibility and commitment to embracing change.
?    Strong relationship-building skills, including the demonstrated ability to build consensus among
diverse stakeholder groups. 
?    Excellent interpersonal, written, and verbal communication skills.
?    Detail-oriented and able to bring structure to an unstructured environment.
?    Proficient in the use of Microsoft Office Suite (Excel, Project, PowerPoint, Word, and Visio) and other
project management tools to aid in the project delivery and data analysis.

Desired Qualifications*

?    Master?s degree in a healthcare related field (e.g. Administration, Business, Engineering, and/or Health
Sciences). 
?    Broad understanding of healthcare trends (e.g. environment, market, payer, provider, regulatory). 
?    Project Management Professional (PMP) certification. 
?    Lean or Six Sigma training/certification and experience with applying lean concepts in planning and operations analysis.

Work Schedule

Hours/ Shift: 40 hours, Day shift

Location: Home Care Services- Post Acute Care Services
