The Wraparound Care Coordinator serves as an advocate/liaison in assisting children/adolescents and their parents/guardians in obtaining services from community resources and other professionals. The Wraparound Care Coordinator is responsible for developing and organizing Child and Family Teams to assess and meet the multiple needs of children who have or at risk of developing behavioral and emotional problems. The Wraparound Care Coordinator coordinates and facilitates Child and Family Team meetings at which the team identifies strengths and needs across all relevant life domains and organizes, initiates, reviews, and revises documented planning. The Wraparound Care Coordinator meets with MH children/adolescent clients and/or their parent/guardians in the home, school, or designated locations to determine and assess needs, transports clients to appropriate appointments with community-based organizations.
Primary Responsibilities;
- Recordkeeping: Complete and submit progress notes, records of interactions (phone calls, meetings, correspondence); track interagency liaison activities on behalf of assigned clients; track progress of treatment plan every 30 days; meet with family to complete Bers-2 report every ninety days and record in State data base; ensure therapist’s completion of annual Positive Behavior Support and Crisis Plan; complete and record residential review every 30 days.
- Facilitate weekly/ monthly meetings with identified Wraparound team to include law enforcement, school personnel, juvenile, mental health professionals, Compass Behavioral Health personnel, etc.; orient them to Wraparound program and engage their involvement to support client; lead and facilitate ongoing meetings with Wraparound team members.
- Research community resources for client support; communicate options to Wraparound team and client; continue ongoing communication, both oral and written, to track and monitor effectiveness of provided support (i.e. residential, peer support program, etc.) Travel to and from client’s homes, other agencies as needed.
- Orient new client to Wraparound and gain consent from appropriate authority to proceed with services; assess needs, expectations and strengths of clients for initial Child and Family Team (CFT) meeting.
- Facilitate residential determination process, researching placement options, completing referral documentation and communicating process to Wraparound team.
- Will be required to achieve productivity standards as set forth and outlined by your manager.
- Maintain compliance with regard to state, federal and Compass Behavioral Health guidelines including but not limited to accurate and timely documentation.
- Other duties as assigned.
Qualifications
Required Qualifications:
- Bachelor’s degree in psychology, social work science or related field, plus two years of experience in a mental health setting or a satisfactory equivalence of education, experience, and training.
- Driving and travel is a requirement of this position. A valid Oregon Driver’s license and acceptable driving record are required.
- Registered as a QMHA through MHACBO.
