PURPOSE:
This position will support the Maryland government programs division, working as a bridge between the member, clinical and administrative staff. The Navigator helps members, families, and caregivers access the health care delivery system through outreach, education, follow-up, coordination support, and resources. Outreach includes both telephonic and community-based outreach, including home-visits with our members. We are looking for an experienced professional to work remotely within Baltimore and surrounding region. While this position is remote, the incumbent will be expected to come into a CareFirst location periodically for meetings, trainings and/or other business-related activities, in addition to traveling throughout the community they serve to support our members.
In addition, the Community Health Navigator participates in community-based events, representing the face of CareFirst and its efforts to promote the health and wellness of our members and their communities.
ESSENTIAL FUNCTIONS:
Act as a liaison to ensure members are connected with appropriate care channels (i.e. pharmacy, insurance, billing, referral) and have the information they need to successfully navigate their care journeys.- Outreach and follow-up with patients in between episodes of care as appropriate to promote a seamless and predictable healthcare experience. Ensure that member went to appointments and assist with coordination for transportation and pharmacy needs.
- Collect all information and relay to Care Coordination team.
QUALIFICATIONS:
Education Level: High School Diploma or GED
Experience: 3 years community relations/outreach/ customer services/ health educator rolePreferred Qualifications:
Crisis Intervention skills
- Skilled in typing and working within various web-based platforms
- Experience with Maryland Medicaid/Medicare recipient population
- Reliable transportation used for member engagement
- Bilingual- fluent in Spanish
- Ability to have a flexible schedule
Knowledge, Skills and Abilities (KSAs)
- Strong written and verbal communication skills.
- Problem-solving skills to tackle various tasks and challenges.
- Must be able to meet established deadlines and handle multiple customer service demands from internal and external customers, within set expectations for service excellence. Must be able to effectively communicate and provide positive customer service to every internal and external customer, including customers who may be demanding or otherwise challenging.
Salary Range: $37,656 - $69,036
